Please call at least 24 hours ahead of time if you need to reschedule to avoid a $65 no-show fee.
 

Established patients:
Arriving 10 or more minutes late will result in a reschedule.

First-time patients:

Be sure to complete and submit your forms two days before your appointment, and arrive 15 minutes early.
Missing forms or late arrival will require rescheduling.
