New patient forms must be completed and sent to ContactCFM@LMGDoctors.com at least TWO business days before your appointment.

If your forms are not received, your appointment will be rescheduled.

Please call at least 24 hours ahead of time if you need to reschedule to avoid a $65 no-show fee.
 

Established patients:
Arriving 10 or more minutes late will result in a reschedule.

First-time patients:

Be sure to complete and submit your forms two days before your appointment, and arrive 15 minutes early.
Missing forms or late arrival will require rescheduling.
